Service Schedule and Terms:

The provider agrees to perform preventive maintenance services twice per year, with scheduled visits in the spring and fall, beginning on ___________________. The client shall provide access to the premises during agreed-upon times.

Fees and Payment:

The client agrees to pay the service provider the amount of $__________ per visit, payable upon receipt of invoice. Additional repairs or parts will be billed separately after prior approval.
