Description of Performance:

The performer agrees to provide musical entertainment at the venue located at 456 Main Street, Los Angeles, CA 90002, on the agreed date and time, including sound check and performance, as specified below.

Performance Date, Time, and Payment:

The performance shall occur on ______________________ at ______________________. The performer shall receive a fee of $__________, payable upon completion of the performance or as otherwise arranged.
