Activity Waiver and Release Form Example

Organizer: Adventure Sports Inc.

Description of the Activity:

Participation in the outdoor rock climbing activity scheduled at Quarry Park, Springfield, Illinois, including all related exercises, equipment, and supervision provided by Adventure Sports Inc.

Acknowledgment and Assumption of Risk:

The participant understands and acknowledges the inherent risks associated with rock climbing, including but not limited to falls, equipment failure, and health risks. The participant voluntarily assumes all such risks and agrees to follow all safety instructions provided by the organizer.

Release and Waiver of Liability:

In consideration of being allowed to participate, the participant releases, waives, discharges, and covenants not to sue Adventure Sports Inc. from any and all liability, claims, demands, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained during the activity, whether caused by negligence or otherwise.

Medical Treatment Authorization:
